Welcome to the Washington State Department of Health site for public record disclosure. This site provides contact information for the Public Record Disclosure Coordinators in the Department of Health divisions and offices. They are responsible for our work in addressing your public record disclosure request.

How to make a request?

To make a Public Record Disclosure request, you may e-mail, call, write, fax or ask for the records in person. The listing below shows e-mail, postal and phone information for each Public Record Disclosure Coordinator.

If you believe records may be in one or more division or office, please send it to just one. Public Record Disclosure Coordinators check with each other when records are likely to be in more than one place. If you know that records may be in different places, please include that information in your request.

To e-mail your request, click on the name of a Public Record Disclosure Coordinator for the division or office where you believe the records are. An e-mail to that Coordinator will appear on your screen.

If you want to make the request in person, contact the Public Record Disclosure Coordinator for directions to the office.

For us to respond as quickly as possible, please provide as much information about the records as you can. We also need information about how to contact you if we have questions and where to send the records.
