State revokes, suspends licenses, certifications, registrations of health care providers

 OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County

In May 2009 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Larissa J. Dixon Murillo (NAC.NC.10092364). Dixon Murillo was convicted of first degree theft.

In June 2009 the Nursing Commission indefinitely suspended the license of registered nurse Laura L. Nicholson (RN.RN.00155113). Nicholson’s license to practice nursing in California was revoked.

In April 2009 the Chiropractic Commission suspended the license of Jeffrey A. Sullivan (CH00002853). Sullivan engaged in inappropriate conduct with a minor, including kissing her on more than one occasion. He pleaded guilty to communication with a minor for immoral purposes.

Clallam County

In May 2009 the Nursing Commission withdrew the statement of charges against registered nurse and advanced registered nurse practitioner Deborah A. Stokke (RN.RN.00051571, ARNP.AP.30001238-ANP).

Clark County

In May 2009 the Secretary of Health dismissed the statement of charges against registered counselor Nahid F. Razzaghy (RC00032818).

In April 2009 the Nursing Commission summarily suspended the license of registered nurse Vickie A. Worthington (RN.RN.00157455). Worthington surrendered her license to practice nursing in Arizona.

Cowlitz County

In June 2009 the Nursing Commission indefinitely suspended the license of licensed practical nurse Ralph G. Snyder (LPN.LP.00055296). Snyder failed to comply with previous terms and conditions set against his license.

King County

In April 2009 the Unlicensed Practice Program the Unlicensed Practice Program ordered Billie S. Barnes to immediately cease and desist from engaging in any and all conduct constituting the practice of medicine in Washington unless she’s properly licensed.

In May 2009 the Nursing Assistant Program suspended the credential of certified nursing assistant Buba J. Bojang (NAC.NC.10026544). Bojang was convicted of communicating with a minor for immoral purposes.

In June 2009 the Nursing Commission summarily suspended the license of registered nurse Raynee J. Frederick (RN.RN.60001541). Frederick’s license to practice nursing in Louisiana was suspended and she remains prohibited from practicing there.

In April 2009 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Joseph N. Nganga (NAC.NC.10088657). Nganga told a patient she needed to die and left a window open in her room, letting cold air in.

In April 2009 the Unlicensed Practice Program ordered Kristine Nguyen, Kim Hieu Nguyen, and Lavender Beauty Salon & Spa to permanently cease and desist from engaging in any and all conduct constituting the practice of massage in Washington until they’re properly licensed. Neither Nguyen nor Nguyen is licensed to practice as a massage therapist in Washington. They incorporated, owned, and operated the business, advertised and provided massages, and did not display the license number of a license holder.

In June 2009 the Medical Commission withdrew the statement of charges against physician Kevin Sloan (MD00029132).

Pierce County

In June 2009 the Registered Counselor Program indefinitely suspended the credential of Billy Lee Jacobs (RC.RC.60040868). Jacobs failed to comply with terms and conditions set against his credential.

In June 2009 the Nursing Assistant Program indefinitely suspended the credential of certified nursing assistant Lillian M. Kroll (NAC.NC.10069892). Kroll borrowed money from a patient.

In June 2009 the Medical Commission indefinitely suspended the license of physician Manuel Posadas (MD00021612). Posadas failed to comply with a previous order.

In June 2009 the Medical Commission entered into an agreed order with physician R. Ivan Zbaraschuk (MD00017853). Zbaraschuk agrees to surrender his license to practice as a physician and surgeon, and never resume practice in Washington.

Snohomish County

In May 2009 the Nursing Commission indefinitely suspended the license of licensed practical nurse Jacquelyne L. Cameron (LP00030487). Cameron failed to comply with a previous order.

In June 2009 the Veterinary Board of Governors summarily suspended the license of veterinarian Johannes Heiter-Krieger (VET.VT.00004401). Heiter-Krieger’s license to practice as a veterinarian in Oregon was revoked for felony conviction of incest and second degree child molestation.

In June 2009 the Nursing Commission indefinitely suspended the license of licensed practical nurse Patricia S. Scheuffele (LPN.LP.00049847). Scheuffele failed to comply with a previous order.

Spokane County

In May 2009 the Nursing Assistant Program suspended the credential of certified nursing assistant Autumn E. Edwards (NAC.NC.10085654). Edwards was convicted of two counts of delivery of a controlled substance and one count of manufacture of a controlled substance.

In May 2009 the Dental Commission withdrew the statement of charges against dentist Allen Eugene Hinkle (DENT.DE.00003570).

In May 2009 the Dental Commission withdrew the statement of charges against dentist John D. Kimball (DENT.DE.00005584).

Thurston County

In June 2009 the Nursing Commission indefinitely suspended the license of licensed practical nurse Andrea R. Wilson (LPN.LP.00050785). Wilson failed to comply with a previous order.

Whatcom County

In May 2009 the Pharmacy Board indefinitely suspended the license of pharmacist Christian P. Long (PHRM.PH.00041549). Long failed to comply with a previous order.

Yakima County

 In June 2009 the Nursing Commission indefinitely suspended the license of registered nurse Kristal L. Donaldson (RN.RN.00148938). Donaldson failed to comply with previous terms and conditions set against her license.

Out of State

 Arizona: In June 2009 the Nursing Commission indefinitely suspended the license of registered nurse Karen M. Manuel (RN.RN.00050356). Manuel’s license to practice nursing in Arizona was suspended.

California: In June 2009 the Medical Commission summarily suspended the license of physician John Brannigan (MD00040755). Brannigan’s license to practice medicine in California was revoked and he remains prohibited from practicing there.

Minnesota: In May 2009 the Nursing Commission summarily suspended the license of registered nurse Matthew D. Lindemann (RN.RN.60014723). Lindemann surrendered his license to practice nursing in Minnesota, and remains prohibited from practicing there.

 Oregon: In May 2009 the Secretary of Health dismissed the statement of charges against registered counselor and chemical dependency professional Sally A. Lambert (RC00008445, CP00000869).
