State revokes, suspends licenses, certifications, registrations of health care providers

 OLYMPIA ¾ The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Office works with boards, commissions and advisory committees to set licensing standards for more than 70 health care professions (e.g., medical doctors, nurses, counselors).

Information about health care providers is on the agency’s Web site. Click on “Provider Credential Search” on the left hand side of the Department of Health home page (www.doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Benton County 

In May 2009 the Medical Quality Assurance Commission modified restrictions on the license of physician Clarence Washington (MD00014637), who had been suspended in connection with alleged sexual misconduct involving a patient. Washington may practice medicine, but may have no contact with the patient with whom he was allegedly involved, may not treat any female patients outside his office or outside hospitals where he has privileges, and must have a female chaperone present when he treats female patients.

Clark County 

In May 2009 the Paramedic Program permanently revoked the credential of David M. Haas (PARA.ES.01173753) after he pleaded guilty to three counts of first-degree child molestation.

 In May 2009 the Board of Pharmacy indefinitely suspended the credential of pharmacy technician Jessica J. Curtin (PTEC.VA.00042943) after the Oregon State Board of Pharmacy revoked her credential in that state for falsifying patient records.

 In May 2009 the Massage Program immediately suspended the credential of massage practitioner Jason T. Lutz (MASS.MA.00020848) after the Oregon State Board of Massage Therapists suspended his credential to practice in that state because of sexual misconduct.

 In May 2009 the Dental Quality Assurance Commission withdrew charges against dentist Josef W. Lubisch (DE00010088).

 In June 2009 the Nursing Assistant Program permanently revoked the credential of James R. Pugh (NAC.NC.10042994) after he was convicted of second-degree manslaughter in connection with a 4-year-old boy’s death.

Island County

 In May 2009 the Pharmacy Program revoked for two years the license of pharmacy technician Kathleen D. Newman (VA00051808) after she tested positive for cocaine.

King County

 In May 2009 the Nursing Assistant Program indefinitely suspended the credential of Jackson M. Muriuki (NAR.NA60008337) for failure to comply with terms of an earlier order.

 In May 2009 the Massage Program issued a cease and desist order, along with a $1,000 fine, to Kim-Sinh T. Nguyen for practicing massage without a license in Washington.

 In May 2009 the Board of Pharmacy indefinitely suspended the credential of pharmacy technician Diane Barger (VA00041056) after she diverted a drug from her workplace.

 In May 2009 the Chemical Dependency Professional Program indefinitely suspended the credential of Robert J.B. Lattimar (CP00003020). Lattimar directed a recovery program that reported treatment had been provided by a person no longer employed there, and where an uncredentialed person provided treatment.

 In May 2009 the Health Care Assistant Program indefinitely suspended the credential of Deanna R. Holmes (HCA.HC.60017467) after she was convicted of driving under the influence.

 In May 2009 the Health Care Assistant and X-Ray Technician programs indefinitely suspended the credentials of Timothy J. Krakovsky (HCA.HC.00148228, XRAY.XT.00006105) after he used a fraudulent prescription to obtain oxycodone.

 In May 2009 the Nursing Care Quality Assurance Commission withdrew charges against licensed practical nurse Darlene Bausch (LP00044484).

Kitsap County

 In May 2009 the Certified Nursing Assistant Program indefinitely suspended the credential of Mary Grace G. Felicisimo (NAC.NC.10072874). She stole money from eight patients and was convicted of third-degree theft.

Okanogan County

 In May 2009 the Nursing Care Quality Assurance Commission issued a 30-month suspension to registered nurse George Menard (RN00151465) for an inappropriate relationship with a patient.

 In May 2009 the Nursing Care Quality Assurance Commission withdrew a statement of charges against licensed practical nurse Delia Mae Severin (LPN.LP.00038246).

Pierce County

 In May 2009 the Board of Pharmacy permanently revoked the credential of pharmacy assistant Carlos Mascarenas (VB00071266) after he diverted hydrocodone, methadone, diazepam, and oxycodone from the pharmacy where he worked.

San Juan County

 In May 2009 the Registered Counselor and Chemical Dependency Professional programs permanently revoked the credentials of Paul R. MacBeth (RC00004492, CP00003096). MacBeth sold heroin, cocaine and other drugs to police officers during an undercover operation. He also allowed a client to live on his property. The client died of a heroin and cocaine overdose.

Snohomish County

 In May 2009 the Health Care Assistant Program withdrew charges against health care assistant Ellen P. Mattison, also known as Ellen P. Wright (HCA.HC.00150115).

Spokane County

 In June 2009 the Nursing Assistant Program indefinitely suspended the credential of Joshua C. Hooper (NAC.NC.10065365) after he failed to comply with an order that put conditions on his license.

 In May 2009 the Registered Counselor and Chemical Dependency programs suspended for at least five years the credentials of Ronnie B. Fowler (RC00032953, CP00003067) because he had a romantic and sexual relationship with a patient.

Thurston County

 In May 2009 the Nursing Assistant Program permanently revoked the credential of Jason A. Koonrad (NAR.NA.00202054) after he was convicted of first-degree child molestation.

Yakima County

 In May of 2009 the Secretary of Health withdrew charges against nursing assistant Don H. Catron (NAC.NC.10061128).

Out of State

 Arizona: In May 2009 the Nursing Care Quality Assurance Commission immediately suspended the license of registered nurse Cathleen C. Wagoner (RN.RN.00142442). The Arizona State Board of Nursing revoked Wagner’s credential to practice in that state.

Nebraska: In June 2009 the Nursing Care Quality Assurance Commission immediately suspended the license of registered nurse Karen Vivian (RN.RN.60020775) after the Nebraska Department of Health and Human Services Division of Public Health suspended her credential to practice in that state.

Oregon: In May 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the license of registered nurse Bethany R. LaMarre (RN00140737), Tigard, Ore. The action took place after LaMarre surrendered her Oregon nursing credentials in lieu of disciplinary action.

In May 2009 the Nursing Care Quality Assurance Commission immediately suspended the license of registered nurse Jocelyn R. DeGroat (RN.RN.00143786), Eugene, Ore., after the Oregon State Board of Nursing suspended her credentials in that state.

In May 2009 the Nursing Care Quality Assurance Commission indefinitely suspended the license of licensed practical nurse Patricia R. Weaver (LPN.LP.00023459), Tualatin, Ore., after the Oregon State Board of Nursing revoked her credential to practice in that state.

Pennsylvania: In May 2009 the Medical Quality Assurance Commission indefinitely suspended the license of physician Samuel L. Krakow (MD60066916), Drexel Hill, Pa. The Commission based its action on a recent suspension of Krakow's license in Pennsylvania. The Pennsylvania State Board of Medicine suspended Krakow’s license to practice in that state because of allegations that he tried to trade prescription drugs for sex.
